Visit us on site to set up your EPR or the EPR for an other person
Set up your EPR on site:
To gain access to the electronic patient record (EPR), you must identify yourself securely, for which you need an electronic identity (eID). An eID is a type of digital ID. You can choose SwissID or TrustID for your eID. Choose one of these eIDs. Then open an account and install the app on your smartphone.
To set up an EPR, you must be at least 18 years old and live in Switzerland.
Please bring all the documents shown below with you when setting up the EPR:
- The login for your eID account (SwissID or TrustID) and the respective eID app
- OASI certificate or health insurance card
- An active smartphone with a valid mobile number
- Access to your personal email account
- Your passport or ID card
Below you will find an overview of our opening centres.
The opening centre will enter your details and set up your personal EPR. Once the account has been opened successfully, you will be able to access and use your EPR at any time via a secure internet connection.
You can also set up your EPR completely digitally at any time.
Set up an EPR for another person on site
As a legal representative (guardian, custodian, support person, etc.), you can set up the EPR for someone else:

Setting up an EPR for someone else currently requires a personal appointment.
Please bring all the documents mentioned below with you to the appointment:
- Proxy certification (family register, decision on guardianship, support, etc.)
- Filled in declaration of consent
- Copy of the ID of the person to be represented
- Own ID
- OASI number of the person to be represented (e.g. on health insurance card)
- Login for your eID account (SwissID or TrustID) and installed eID app
- Active smartphone with a valid mobile number
- Access to your personal email account
Make an appointment at the EPR opening centre "Central Services of Sanela". You will find the link for registration below.
The opening centre enters the details of the person to be represented and then sets up the EPR of that person. After successfully setting up the record, you can access and use the EPR at any time via a secure internet connection.
You can also set up an EPR for another person completely digitally at any time
